Key Responsibilities\n

The Licensed Clinical Social Worker will be responsible for a range of vital tasks to support clients and their families, which include:

• Conducting thorough evaluations and assessments of clients' mental, emotional, and social needs, ensuring a holistic understanding of their situation.

• Participating in the development, implementation, and monitoring of Individualized Family Service Plans (IFSP), adapting strategies as necessary to meet changing circumstances.

• Building strong, trusting relationships with clients, families, and interdisciplinary team members to foster a supportive and healthy environment.

• Providing evidence-based interventions that utilize Natural Learning Environment Practices, ensuring that clients receive care that aligns with their developmental needs and family priorities.

• Offering valuable information and support to families, empowering them to enhance their loved one’s wellbeing and quality of life.

• Maintaining compliance with all Federal and State regulations to uphold the highest standards of care and ethical practice.

• Regularly documenting client interactions, including progress notes and outcome measures, to ensure consistent treatment delivery and data collection for quality assurance.

Patient Population / Client Focus\n

Your primary focus will be on patients within the hospice care framework, which encompasses a diverse group of individuals facing end-of-life issues. This includes supporting patients with terminal illnesses and their families, guiding them through emotional complexities, and enhancing their understanding of available resources and services. Qualifications\n

To be considered for this role, applicants must meet the following qualifications:

• A Bachelor's Degree in Social Work or related field is required.

• Current licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.

Compensation and Benefits\n
