Key Responsibilities\n

• Conduct thorough psychosocial assessments to identify the emotional, social, and environmental factors affecting patients and their families.

• Develop, implement, and evaluate individualized treatment plans that utilize appropriate social work interventions, ensuring alignment with healthcare goals.

• Guide and educate patients and families regarding adaptive strategies for managing illnesses and navigating their unique life situations.

• Facilitate effective discharge planning by coordinating with healthcare providers, including physicians and third-party payers to streamline the transition process.

• Provide a range of therapeutic services, including short-term crisis intervention and long-term support through individual, group, and family therapies.

• Act as a resource for other health care professionals, offering training and educational resources related to psychosocial oncology practices.

• Engage in community outreach by promoting and strengthening local services and programs designed to assist cancer survivors.

• Participate in an on-call rotation to manage after-hour calls, ensuring continuous support for patients in need.

• Contribute to the professional development of the organization through participation in educational conferences and training sessions related to oncology social work.

Patient Population / Client Focus\n

The patients you will serve primarily include individuals diagnosed with terminal illnesses, particularly in a hospice setting. Qualifications\n

• A Bachelor's degree in Social Work (BSW) from an accredited institution is required.

• A valid LCSW license in New York State is mandatory.

Preferred Experience\n

Compensation and Benefits\n
