Key Responsibilities\n

• Provide direct clinical services to clients dealing with major mental health challenges, ensuring tailored interventions in line with their unique needs.

• Conduct thorough, evidence-based assessments and screenings to identify clients’ health needs and develop comprehensive care plans.

• Facilitate individual, group, and family therapy sessions, providing clients with the necessary tools and strategies to navigate their conditions.

• Collaborate with a multidisciplinary team to ensure an integrated approach to client care, sharing insights and fostering communication between all stakeholders.

• Maintain accurate and timely documentation of client interactions, treatment plans, and progress, adhering to both ethical guidelines and the organization’s protocols.

• Support clients during times of crisis and provide intervention as necessary, demonstrating empathy and professional judgment.

• Participate in continuous quality improvement initiatives, contributing to team committees aimed at enhancing service delivery.

Patient Population / Client Focus\n

The primary focus of this role will be on clients in hospice care, many of whom are experiencing significant emotional and psychological distress due to critical illness. You will play a pivotal role in helping these individuals and their families understand and navigate their emotional responses, work through grief, and access the support systems they need. This requires not only a solid foundation in social work practice but also a deep sense of compassion and sensitivity to individual and family experiences at a time of vulnerability.

Qualifications\n

• A Bachelor’s Degree in Social Work from an accredited institution is required.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in Illinois is mandatory.

• A minimum of 2 years of relevant experience working in clinical settings, ideally in hospice, mental health, or similar environments.

Compensation and Benefits\n

This position offers a competitive salary ranging from USD 70,000 to 85,000 per year, commensurate with experience and qualifications.
