Key Responsibilities\n

• Conduct comprehensive assessments of seniors' psychosocial needs, factoring in emotional, social, and mental health aspects to tailor individualized care plans.

• Collaborate closely with clients and their families to establish achievable care goals that prioritize their well-being and preferences.

• Provide counseling and emotional support to help clients navigate significant life transitions and challenges, serving as a trusted guide.

• Advocate vigorously for clients’ rights, assisting them in accessing important community resources and services needed for their care.

• Engage in regular communication with the interdisciplinary team, including medical professionals and caregivers, to ensure holistic care delivery.

• Visit clients digitally to monitor their well-being, assess living conditions, and proactively address any potential concerns or safety risks.

• Facilitate effective communication between family members involved in care, assisting in conflict resolution and fostering collaborative relationships.

• Maintain accurate records of client assessments, progress notes, and care plans, ensuring compliance with ethical and regulatory standards.

Patient Population / Client Focus\n

This position primarily focuses on seniors dealing with chronic illnesses or terminal conditions within the hospice setting. Your expertise will be instrumental in addressing their unique emotional and psychological needs, providing them and their families with the support and guidance necessary during this pivotal stage of life.

Qualifications\n

• A master’s degree in Social Work from an accredited institution is required.

• Current and active state licensure as a Licensed Clinical Social Worker (LCSW).

• A minimum of two years of relevant experience in a social work capacity, preferably within a geriatric or community center setting.

• Demonstrated knowledge of social work principles, particularly as they relate to aging and hospice care.

• Exceptional assessment, counseling, and problem-solving skills.

Compensation and Benefits\n

The salary for this position ranges from USD 60,000 to 100,000 annually, commensurate with your experience and qualifications.
