Key Responsibilities\n

• Establish rapport with patients and their families, using empathetic communication skills and effective listening techniques to foster an open dialogue.

• Collaborate with a multidisciplinary team, including medical providers and other behavioral health professionals, to coordinate holistic treatment plans tailored to each patient.

• Conduct thorough evidence-based screenings to identify mental health issues such as depression, anxiety, and substance abuse, determining the need for more detailed assessments when appropriate.

• Create and manage a caseload of patients, providing timely follow-ups and brief interventions that support their recovery journey.

• Utilize our electronic medical records system to promptly document patient encounters, ensuring all records are maintained accurately within 48 hours of each interaction.

• Develop comprehensive integrated care plans, facilitating the linkage of patients to necessary services and programs that align with their treatment goals.

• Maintain strict adherence to patient privacy and confidentiality throughout all communication, documentation, and data handling.
\n
• Participate actively in supervisory and quality improvement meetings, contributing to the overall efficacy of our treatment approaches.

• Respond promptly to inquiries from patients and their families regarding appointments and treatment concerns, reinforcing BCHC's commitment to accessible support.

• Identify and build connections with community resources, including support groups and vocational services, to further empower patients in achieving their objectives.

Patient Population / Client Focus\n

In this role, you will primarily serve individuals experiencing substance use disorders, many of whom may be from underserved or marginalized backgrounds. A significant portion of our patients may also present with co-occurring mental health issues. Your expertise will not only be crucial in assessing and addressing their immediate needs but also in supporting their holistic recovery journey and connecting them with long-term resources.

Qualifications\n

• A Master's degree in a relevant Behavioral Health field.

• Active LCSW license is required.

• Current certification in CPR or Basic Life Support (BLS).

• A minimum of 3 years of experience working in the Behavioral Health field is essential.
