Work Environment

This role is based onsite in the community, where you will meet clients in their own homes, allowing for flexibility and the opportunity to create personalized care plans tailored to their specific environments. Key Responsibilities

• Conduct comprehensive evaluations of clients, including assessment of their medical history and physical capabilities, to determine appropriate individual treatment plans.
• Communicate evaluation findings and treatment plans effectively with clients and their families, fostering an inclusive environment for setting therapy goals.
• Implement evidence-based treatment protocols adjusting plans based on clients' progression and feedback, utilizing progressive techniques such as manual therapy, therapeutic exercises, and modalities as needed.
• Document all findings, treatment progress, and recommendations meticulously to ensure compliance with healthcare regulations and organizational standards.
• Collaborate with other healthcare professionals to discuss patient care strategies, sharing insights and drawing on multidisciplinary expertise.
• Engage clients in their treatment by teaching distinct exercises and guiding them through recovery, preventing future injuries.

Patient Population / Client Focus

Your primary focus will be working with home care patients across a varied demographic spectrum, including athletes recovering from sports injuries, elderly individuals managing chronic conditions, and post-surgical patients. Each client will benefit from tailored therapy approaches aimed at maximizing their recovery outcomes.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited program.
• Valid physical therapy license in the state of Florida.
• Strong assessment and diagnostic skills with a commitment to delivering high-quality patient care.
• Excellent communication and interpersonal skills, with a focus on patient engagement and education.

Preferred Experience

• Minimum of two years of experience in outpatient or home care settings.
• Familiarity with electronic health records (EHR) systems.

Compensation and Benefits
