Key Responsibilities\n

• Conduct in-depth evaluations of clients to assess their mental health needs, informing personalized treatment plans.

• Facilitate effective communication among healthcare team members, clients, families, and guardians, ensuring that all parties are updated on progress and treatment objectives.

• Document client interactions and treatment plans meticulously in our Electronic Medical Record (EMR) system, ensuring compliance with legal and ethical standards.

• Conduct regular home visits to provide in-home assessments and observations, fostering a deeper understanding of clients' environments.
\n
• Actively participate in Interdisciplinary Team Meetings for comprehensive case management and collaborative decision-making.
\n
• Generate monthly and quarterly progress reports that summarize client outcomes, treatment effectiveness, and recommendations for ongoing care.

• Provide training and support for new team members and staff regarding program execution and documentation procedures.
\n
• Continuously evaluate and adjust treatment approaches based on client feedback and emerging best practices.

• Engage with the broader community to bridge gaps in mental health awareness and accessibility, promoting mental wellness initiatives.

Qualifications\n
