Key Responsibilities\n

• Conduct comprehensive assessments to determine the therapeutic needs and goals of each client.

• Engage in open communication with supervisors, direct care teams, and family members regarding patient progress and evolving needs.

• Develop and maintain a tentative schedule for appointments, home visits, and monitoring tasks to track client advancements in therapy.

• Coordinate in-home visits, including observations, chart reviews, staff interviews, and data analysis to support client care.

• Ensure adherence to and effective implementation of individualized program plans.

• Actively participate in Interdisciplinary Team Meetings as well as other relevant discussions to share insights and enhance patient outcomes.

• Prepare detailed monthly and quarterly progress notes to document client developments and therapeutic interventions.
\n
• Organize and lead training sessions aimed at equipping team members with the necessary skills to implement program plans and maintain documentation standards.
\n
• Provide ongoing support through follow-up visits and observations, ensuring continuity of care for clients.
\n
• Maintain accurate records that are accessible to authorized personnel and stakeholders.
\n
• Function as a liaison to foster relationships with community resources and mental health organizations to support individual and organizational goals.

• Assist in developing training materials and protocols to enhance program efficiency and staff competence.

Compensation and Benefits\n
