Key Responsibilities\n

• Provide thorough psychosocial assessments for patients and their families, ensuring detailed understanding of their emotional, social, and practical needs. This includes conducting interviews and assessing the need for additional support and interventions.

• Develop individualized treatment plans tailored to each patient's unique circumstances, integrating family dynamics and cultural considerations.

• Ensure timely and effective documentation of all clinical interactions within 72 hours, adhering to the highest standards of practice and legal regulations set forth by relevant licensing boards.

• Work collaboratively with the hospice care team, including physicians and nursing staff, to coordinate comprehensive care plans aimed at enhancing patient quality of life.

• Facilitate family meetings and support groups, providing education and emotional support to help families cope with the challenges of end-of-life care.

• Effectively manage conflicts and difficult conversations by applying best practices in communication to achieve positive outcomes for both patients and their families.

• Maintain productive relationships with community resources and agencies to enhance support systems for clients.

Patient Population / Client Focus\n

In this role, you will primarily engage with terminally ill patients and their families who require compassionate support and guidance through the hospice care process. This includes patients with various life-limiting conditions such as cancer, cardiac diseases, and neurodegenerative disorders. You will play a critical role in helping clients navigate their emotional and practical needs during this sensitive time.

Qualifications\n

• A Master’s Degree in Social Work from an accredited program is required, reflecting a rigorous academic foundation in clinical practices.

• A minimum of 2 years of clinical experience in a behavioral health or social service setting, specifically within hospice care or a related healthcare environment.
