Key Responsibilities\n

• Conduct comprehensive virtual psychosocial assessments to thoroughly evaluate clients' strengths, needs, and potential barriers to care.

• Deliver evidence-based therapeutic interventions, including individual and group therapy, tailored specifically to the unique needs of hospice clients and their families.

• Collaborate effectively with interdisciplinary teams, including physicians, nurses, and other healthcare professionals, to coordinate integrated care plans that enhance overall patient wellness.

• Advocate for client rights, ensuring access to necessary community resources, support services, and emotional essentials during their care journey.

• Document all client interactions, treatment plans, and progress clearly and accurately in electronic health records, maintaining compliance with confidentiality regulations.

Qualifications\n

• A Master’s degree in Social Work from an accredited university is required.

• Current licensure as a Licensed Independent Clinical Social Worker (LICSW) in the state of Washington.
