Key Responsibilities\n

• Conduct thorough virtual psychosocial assessments to identify clients' strengths, emotional needs, and areas for therapeutic intervention.

• Design and implement evidence-based treatment plans tailored to the individual needs of each client, emphasizing a person-centered approach.

• Facilitate virtual individual and group therapy sessions, utilizing modalities such as Cognitive Behavioral Therapy (CBT) and Dialectical Behavior Therapy (DBT), depending on client needs.

• Collaborate closely with a multidisciplinary team, including physicians, nurses, and other healthcare professionals, to coordinate integrated care for clients in hospice settings.

• Advocate for clients' rights, ensuring they have access to necessary resources, supports, and entitlements within the community.

• Accurately document client interactions, treatment progress, and outcomes within electronic health records (EHR) in compliance with HIPAA and ethical standards.

• Participate in regular team meetings, case conferences, and continuing education to remain current with best practices and enhance clinical skills.

Qualifications\n
