LPN

The Licensed Practical Nurse (LPN) in a Post Acute Long-Term Care (LTC) setting provides comprehensive nursing care consistent with LPN responsibilities. The role involves observing and reporting resident conditions, administering medications and treatments, maintaining accurate records, assisting with assessments and care plans, supervising nursing staff, and collaborating with physicians and other healthcare professionals to ensure high-quality resident care during evening shifts. Responsibilities:

• Observe and report symptoms and conditions of residents accurately.
• Administer medications and treatments as prescribed by physicians.
• Count narcotics and maintain detailed records.
• Maintain and update records reflecting resident condition, medication, treatments, and progress.
• Transcribe physician orders and place drug orders as required.
• Assist with nursing history, assessments, Minimum Data Set (MDS), and care plan development.
• Assist in orientation and supervision of nursing staff and directly supervise CNAs.
• Perform direct resident care and assume Charge Nurse responsibilities when necessary.
• Coordinate CNA assignments and manage communication with physicians and family members.
• Book tests, labs, x-rays, and medical appointments; assist physicians during rounds.
• Perform resident admissions and discharges and notify appropriate parties of changes in resident status.
• Document nursing notes and update care plans regularly.
• Participate in ongoing in-service education and adhere to facility policies and nursing practices.

• Current licensure as a Licensed Practical Nurse (LPN) in the state of Massachusetts.
• Graduate of an accredited school of nursing.
• Current CPR/AED certification.
• Ability to recognize changes in resident condition and take appropriate action.
• Strong English communication skills, both written and verbal.
• Physical ability to perform duties including lifting up to 50 pounds, standing for extended periods, bending, squatting, kneeling, walking, and reaching.

• Experience in post-acute or long-term care nursing.
• Previous supervisory or charge nurse experience.
• Familiarity with electronic medical records and documentation systems.

• Must be able to cope with physical and mental job stress.
• Responsible for on-call communication with physicians and family members as needed.
• Ability to work effectively in a fast-paced long-term care environment.

• Evening shift schedule, typical for post-acute long-term care nursing roles.
• Shift assignments and hours determined by facility needs and nursing management.
