LPN

The Licensed Practical Nurse (LPN) in a post-acute long-term care setting provides skilled nursing care consistent with LPN responsibilities. This role involves observing and reporting resident conditions, administering medications and treatments, maintaining accurate records, assisting with care planning and assessments, and supervising nursing staff. The LPN collaborates closely with physicians, nursing teams, and families to ensure high-quality care and supports facility operations including charge nurse duties when necessary. Responsibilities:

• Observe and report symptoms and conditions of residents.
• Administer medications and treatments as prescribed by physicians.
• Count narcotics and maintain accurate records.
• Maintain records reflecting resident condition, medication, treatments, and progress.
• Transcribe physician written and telephone orders.
• Place drug orders and assist with nursing history, assessments, MDS, and care plans.
• Assist in orientation and supervision of nursing staff and CNAs.
• Perform direct resident care as necessary.
• Participate in ongoing in-service education.
• Assume Charge Nurse responsibilities when necessary.
• Prepare and coordinate CNA assignments.
• Communicate with physicians, book tests, labs, x-rays, and medical appointments.
• Assist physicians during rounds and prepare charts for signatures.
• Check meal tray accuracy.
• Perform resident admissions and discharges.
• Notify MD/NP and family members/HCP of any changes in resident status or treatment.
• Document nursing notes and update care plans as required.

• Current licensure as a Licensed Practical Nurse (LPN) in the state of Massachusetts.
• Graduate of an accredited school of nursing.
• Current CPR/AED certification.
• Ability to recognize changes in resident condition and take appropriate action.
• Proficient in reading, writing, and speaking English clearly and legibly.
• Physical ability to lift up to 50 pounds, push/pull 40 pounds, stand for up to 4 hours, bend, squat, kneel, walk, and reach up to 4.5 feet.

• Experience in post-acute or long-term care nursing.
• Charge nurse or supervisory experience.

• Ability to cope with physical and mental job stress.
• May be required to assume charge nurse responsibilities.
• Work involves direct supervision of CNAs and coordination of nursing care.

• Day shift schedule.
• Shift assignments may be determined by unit manager or facility needs.
