Key Responsibilities\n

• Conduct intake assessments and diagnostic evaluations for new patients, systematically documenting findings and treatment recommendations in coordination with primary care providers.

• Facilitate close collaboration with primary care providers to address and manage patients' mental health and psychosocial issues, actively participating as a member of the patient-centered care team.

• Develop personalized treatment plans, set measurable goals, and monitor progress to adjust care strategies as necessary.

• Utilize evidence-based practices to conduct follow-ups with patients, ensuring continuity of care and assessing the effectiveness of interventions.

• Assist in identifying patients at risk of worsening mental health conditions and craft suitable relapse prevention plans.

• Maintain comprehensive documentation of patient progress, treatment outcomes, and communications, ensuring compliance with ethical and legal standards.

• Consult and provide guidance to clinic management about behavioral health services, continuous improvement opportunities, and program evaluation metrics.

Qualifications\n

Compensation and Benefits\n

The salary range for this position is competitive, starting at $87,000 and going up to $105,000 annually, based on experience and qualifications.
