Bilingual Case Manager

About the Opportunity: The Latinx Family Empowerment Specialist (LFES) is primarily responsible for supporting families and parents while maintaining and managing a caseload through home visits, phone support, basic needs/item drop offs, off-site visits, and parent education. The LFES will provide differentiated in-home support, based on client needs, while acting as a positive role model. 70 client contact hours per month is required between crisis intervention and family stabilization clients. The LFES is also responsible for maintaining clear and accurate records, executing goals as stated for funded programs. The LFES works closely with LOP and Nursery staff in the support of a family’s needs. The LFES may travel between different outreach locations to support, based on client needs. The LFES will support the food pantry, as needed.

REQUIREMENTS :

Minimum, bachelor’s degree in a social service-related field; pay is commensurate with education and experience. Bi-Lingual in English and Spanish Valid Driver’s License Reliable transportation

EXPERIENCE:

Must be at least 21 Minimum one year verified working experience with children and families Paid/unpaid home visiting experience preferred

DUTIES & RESPONSIBILITIES

Provide in-home support through crisis intervention, and case management, parent education, based on the needs of the family. Assess families for appropriateness for either Family Stabilization or Crisis Intervention track, while offering additional referrals and resources, as needed. Complete 70 service hours per month with families. Conduct at least one home visit per family each month, and more often if needed. Contact families that have utilized the nursery through the first call process within 72 hours of discharge from the Nursery. Demonstrate a welcoming, mission-focused, and professional presence to clients, staff, students, volunteers, and community partners. Support with LOP food pantry. Identify a safety partner at each regional location. Utilize a shared calendar to consistently document and communicate home and field appointments as well as schedule changes; include date, time, name, address and contact number, and notify your partner after the last appointment of the day. Support new staff, volunteers, and students through collaboration, shadowing, and training, as needed. Communicate progress and concerns with supervisor bi-weekly through supervision and as needs arise. Complete Pre – Mid – Post assessment and treatment plans with families to assess growth and redefine goals. All assessments should be completed based on grant guidelines. Daily client notes should be entered into the database within 24 hours of client contact. Provide item drop offs to families to support the family with concrete needs, as needed. Complete community event form and enter into database within 72 hours. Lead parent education groups (PEG) based on client needs. Communicate PEG plans with team and supervisor one month in advance. Attend agency meetings such as: All-Staff Meetings, team meetings, interdisciplinary team meetings, committees, and other meetings as determined by agency needs. Network with other agencies on a regular basis. Collaborate with the Nursery team to support clients and one another. Other duties as assigned.

KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:

Knowledge: Possesses comprehensive knowledge of crisis intervention principles and effective support strategies for parents and children navigating crises. Demonstrates expertise in applying strength-based philosophy, solution-focused goal setting, child development knowledge, and parent coaching techniques tailored to developmental stages from birth through 12 years. Understanding of supporting clients through crises that may arise. Skills: Exceptional, bi-lingual verbal and written communication skills, with the ability to actively listen, express ideas clearly and professionally, and foster productive conversations with families, staff, and management. Always exercise discretion and independent judgement. Skilled in collaborative problem-solving with clients and interdisciplinary teams to identify solutions. Strong organizational skills that ensure effective case management and timely follow-up. Abilities : Able to empower and support family growth by providing strength-based interventions and making appropriate referrals for families in crisis. Demonstrates a calm, empathetic, and non-judgmental approach in handling sensitive situations with children and families. Proficient in using computer software and database systems and staying current with technological advancements. Capable of delivering engaging presentations to small groups. Holds a valid driver’s license with an acceptable driving record, enabling travel to various locations to meet with families. Working Conditions: Role consists of some office time as well as services in the field, requiring reliable transportation. In-home environments may be noisy. Occasional exposure to bodily fluids or other potential hazards may occur. Office work is in a temperature-controlled, well-lit building, often shared with colleagues, students, and volunteers.
