Key Responsibilities\n

• Facilitate counseling sessions via virtual platforms, providing emotional support and guidance tailored to the individual's needs.

• Create and implement structured treatment plans that address clients’ unique psychological challenges, ensuring measurable progress towards their goals.

• Assist clients in developing daily living skills, such as shopping, meal planning, and cooking, while encouraging independence.

• Provide medication management support, including timely reminders for refills and adherence monitoring, following guidelines specified by program sites.

• Support clients in accessing and utilizing public transportation to attend medical appointments, ensuring they maintain crucial healthcare connections.

• Maintain a safe environment for clients by observing their mood and behavior, implementing safety protocols, and serving as a positive role model.

• Document all client interactions accurately, including treatment updates, client progress, and compliance with legal and regulatory standards, in accordance with agency policies.

• Participate as an active team member, communicating effectively with colleagues to enhance service delivery and client outcomes.

Qualifications\n

• A Master’s degree in counseling, clinical psychology, social work, or a related field is essential.

• Current licensure as an LCSW, LMFT, or LPCC in the state of California is mandatory; candidates with associates-level licenses cannot be accepted due to Medicare billing restrictions.

• Strong technical skills, particularly proficiency with Microsoft Office Suite and telehealth platforms.

• The ability to work independently in a fast-paced environment, managing multiple cases effectively.

Preferred Experience\n

• Experience working with geriatric populations is preferred, but not required.
