Population Health Outreach Specialist
Corewell Health
Corewell Health is seeking a Population Health Specialist to outreach to patients with gaps in chronic disease management and preventive care. You will support health status improvement through access to services, education, and wellness resources while reinforcing the patient/primary care provider relationship. Responsibilities include using EMR tools and payer portals to identify gaps, tracking outreach, and collaborating with practice teams. Travel to ambulatory sites is required. #J-18808-Ljbffr
