Work Environment\n

As a remote workplace, you will have the flexibility to manage your schedule, fostering a healthy work-life balance while ensuring your commitment to client care. Standard working hours are Monday to Friday, 8:00 AM to 4:30 PM, with potential occasional evening and weekend hours to accommodate client needs. You may also be required to be on-call to respond to urgent situations, ensuring that our clients receive uninterrupted support. Flexibility and adaptability in your work schedule are essential to effectively meet the continuous care demands of your clients.

Key Responsibilities\n

• Conduct detailed assessments of clients' psychosocial needs, focusing on emotional, mental, and social aspects that influence their health and well-being.

• Collaborate with clients and their families to establish personalized care plans that address specific goals and support their aspirations for achieving emotional stability.

• Provide direct counseling services and emotional support to seniors and their families, enabling them to navigate life’s transitions and personal challenges.

• Advocate for clients’ rights and access to vital community resources, ensuring that they benefit from appropriate support services.

• Work in tandem with multidisciplinary teams, including healthcare professionals, caregivers, and nurses, to coordinate comprehensive care efforts.

• Conduct regular client visits to evaluate their living conditions and overall well-being, proactively addressing safety concerns and necessary interventions.

• Facilitate communication and conflict resolution among family members involved in the client’s care, promoting a collaborative support network.

• Educate clients and families about relevant community resources and support programs, enhancing clients' engagement and quality of life.

• Maintain meticulous documentation of client interactions, assessments, and progress notes in accordance with regulatory and ethical standards.

• Stay updated on current social work practices, policies, and ethical standards to ensure compliance and excellence in service delivery.

Your primary focus will be on senior clients and their families, addressing the unique challenges that often accompany aging, including mental health concerns like anxiety and depression. Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).

• At least two years of practical experience in a social work role, ideally in a community health center or a geriatric-focused setting.

• Strong understanding of social work principles and aging-related issues, along with resources available to the elderly population.

Compensation and Benefits\n

This role offers a competitive salary ranging from $90,000 to $100,000 per year, commensurate with your experience and qualifications.
