Job Summary
We are seeking an experienced Intake Coordinator for a direct hire opportunity with a growing home health care agency in Boca Raton, FL. This role is ideal for a detail-oriented healthcare customer service professional who enjoys being the first point of contact for patients, families, referral sources, physicians, and healthcare partners. The Intake Coordinator will manage the referral and admission process from start to finish, ensuring patient information, insurance verification, authorizations, and required documentation are handled accurately and efficiently. This is a meaningful opportunity to support compassionate, high-quality home health services that help patients remain safely and comfortably in their homes. Key Responsibilities - Receive, review, and process incoming patient referrals from hospitals, physicians, case managers, facilities, and other healthcare referral sources.- Verify insurance eligibility, benefits, authorizations, coverage requirements, and payer guidelines for Medicare, Medicaid, and commercial plans.
- Gather and review patient demographic, clinical, insurance, and referral documentation for accuracy and completeness.
- Coordinate admissions with clinical staff, scheduling teams, patients, families, and referral partners to support timely starts of care.
- Enter, update, and maintain patient information and referral activity within the EMR system.
- Follow up on missing documentation, physician orders, intake issues, and escalations in a timely and professional manner. Compensation and Benefits - Salary range: $50,000 to $60,000 per year.
