Position: Registered Nurse (RN) - Home Health

Overview

This role involves coordinating and directing the provision of home nursing care according to physician orders, based on agency policies and procedures. Applicants should possess the competency required for the effective application of the nursing process.

Responsibilities

Provide home nursing care in accordance with physician orders.
Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
Conduct home visits and participate in periodic office meetings.
Complete documentation within 24 hours of patient appointments.
Transport necessary equipment and supplies to patient homes.

Travel Requirements

Home visits will require reliable transportation. Must be willing to travel up to 75 miles from the assigned clinic. Mileage will be reimbursed at the IRS standard rate. The commute to and from the first and last patient are not billable.

Work Schedule

Shift: Day 5x8-Hour (08:00 - 17:00)
Weekends may be required, varying by location and profession.
On-call requirements vary by location and profession.

Patient Care

Average number of patients per day: 5-10.
Types of cases include Start of Care, Wound Care, Follow-Up Visits, etc.

Qualifications

Minimum of one full year of Adult Home Health or Hospice experience required.
First-time applicants are not accepted; recent specialty experience is needed.
Proficiency in HomeCare HomeBase (HCHB) EMR and OASIS within the last 6 months.
Current CPR and BLS certifications are required.

Location

This position covers the Greater Harrisburg area, with a focus on home health care.

Additional Information

Expected weekly productivity points: 30, which corresponds to approximately 40 hours per week.
No guaranteed hours; opportunities to float to a nearby clinic may be available if patient census is low.
Home Health Checklist may be required.
