Bilingual Family Wellness Case Manager
Identity, Inc.
Identity, Inc. is seeking a Case Manager to assess families' needs and coordinate services within our Wellness Center, including assessments, follow-up surveys, and action plans. You will work with school partners, maintain case files, enter data, and provide referrals and mentoring to youth and families.
