Key Responsibilities\n

• Conduct comprehensive psychosocial assessments for patients and families to determine emotional, social, and environmental strengths and challenges related to their health status.

• Implement and formulate individualized discharge plans, utilizing a variety of social work interventions aimed at reducing stress and promoting patient well-being during and after hospitalization.

• Collaborate closely with a multidisciplinary healthcare team to develop care plans that involve input from patients and families, ensuring holistic support during the treatment process.

• Facilitate smooth discharge transitions by coordinating with healthcare providers, insurers, and community resources to create practical discharge strategies that align with ongoing patient needs.

• Employ crisis intervention strategies while also offering both brief and long-term therapies that cater to individual, group, and family needs.

Qualifications\n

• A Bachelor’s Degree in Social Work from an accredited institution is required.

Compensation and Benefits\n

The salary for this full-time position ranges from USD 70,000 to 100,000 annually, commensurate with experience.
