Position Summary Empowering Minds is seeking a motivated, outgoing, and highly organized Community Outreach Coordinator to expand the agency's presence within the communities we serve and develop strong relationships with referral partners, community organizations, and potential clients. The Community Outreach Coordinator will be responsible for identifying new referral opportunities, establishing and maintaining relationships with community partners, promoting the agency's behavioral health services, participating in community events, supporting the referral and intake process, and increasing community awareness of available mental health rehabilitative services. The position will support outreach throughout Baltimore City/County, Anne Arundel County, Harford County, and Prince George's County , with travel between community locations as needed. Essential Responsibilities Community Outreach & Relationship Development Develop and expand the agency's community referral network to increase appropriate client referrals. Identify and establish relationships with hospitals, healthcare providers, schools, community organizations, social service agencies, faith-based organizations, senior programs, treatment providers, and other community partners. Maintain consistent communication and follow-up with existing referral sources. Build professional relationships that encourage continued referrals and collaboration. Increase awareness of the agency's behavioral health services within target communities. Identify emerging community needs and opportunities for agency services. Maintain current knowledge of community resources and referral organizations. Marketing & Agency Promotion Represent the agency professionally at community meetings, networking events, health fairs, resource fairs, and other outreach activities. Distribute brochures, flyers, promotional materials, and other agency information. Conduct presentations to community organizations, referral sources, potential clients, and other stakeholders. Clearly explain the agency's services, eligibility requirements, referral process, and available programs. Assist with development and implementation of community outreach and marketing initiatives. Utilize social media and appropriate marketing platforms to increase awareness of agency services. Provide feedback regarding community response to agency programs and outreach efforts. Referral & Intake Support Respond promptly to inquiries and referrals. Explain the referral and intake process to potential clients, families, and referral sources. Conduct preliminary screening to determine potential eligibility for services. Collect required client information, eligibility documentation, assessment information, and intake documents. Complete Intake Assessments as assigned and within established agency requirements. Follow up with referral sources and potential clients regarding the status of referrals. Coordinate communication between outreach activities and the appropriate internal program staff. Maintain accurate documentation of outreach activities, referrals, contacts, and follow-up efforts. Community Events & Networking Attend and participate in community events throughout the agency's service area. Develop an outreach calendar identifying networking opportunities, community events, presentations, and referral-source visits. Assist with planning and coordinating agency-sponsored events. Establish a visible and professional presence for the agency in the community. Develop relationships with community leaders and organizations that can increase access to behavioral health services. Compliance & Confidentiality Maintain strict confidentiality of all client information in accordance with agency policies and applicable privacy requirements. Follow agency procedures regarding client information, referrals, documentation, and intake activities. Maintain professional boundaries when interacting with clients, families, referral sources, and community partners. Complete required documentation accurately and timely. Participate in staff meetings, training, supervision, and quality improvement activities as required. Performance Expectations Consistently develop new community relationships and referral opportunities. Maintain productive relationships with existing referral sources. Generate and appropriately track referrals. Follow up with referrals in a timely manner. Maintain accurate outreach and referral records. Regularly participate in community outreach activities. Demonstrate knowledge of the agency's services and referral requirements. Meet established outreach and referral activity goals. Represent the agency positively and professionally. Demonstrate initiative and work independently while maintaining communication with agency leadership. Minimum Qualifications Bachelor's degree in Human Services, Social Services, Behavioral Health, Marketing, Public Relations, Communications, or a related field. Minimum of two years of experience in human services, behavioral health, community outreach, marketing, public relations, or a related field. Excellent written, verbal, and interpersonal communication skills. Demonstrated ability to establish and maintain professional relationships. Experience with community networking and knowledge of community resources, preferably within the agency's target communities. Strong organizational and time-management skills, including meeting multiple deadlines. Ability to manage multiple assignments, priorities, and projects simultaneously. Proficiency with Microsoft Word, Excel, PowerPoint, email, and other computer applications. Knowledge of social media and basic marketing platforms. Strong attention to detail. Ability to communicate information clearly and concisely. Ability to maintain a high level of confidentiality. Self-motivated, enthusiastic, professional, and dependable. Ability to work independently and as part of a team. Willingness and ability to travel throughout the agency's service area. Preferred Qualifications Experience working in a behavioral health or mental health organization. Experience with community-based mental health rehabilitation services. Existing relationships with community organizations and referral sources. Experience conducting presentations and community education. Experience with client intake, screening, or referral coordination. Experience using electronic health records or client management systems. Knowledge of behavioral health resources and social service systems. Experience developing community partnerships and referral networks. Travel Requirements This is a community-based position requiring regular travel between outreach locations, referral sources, community organizations, and agency locations. A company vehicle is provided for approved business travel.
