REQUIREMENTS

• Active FL license
  
• Active DEA
  
• Active ACLS or BLS
• Completed Lee Attestation Form
  
• Clean NPBD report (within the last 6 months)
  

DUTIES & RESPONSIBILITIES

• Provide cardiology coverage at three campuses.
  
• Perform rounding duties on patients from 8:00 AM to 5:00 PM on assigned dates.
  
• Provide call coverage as scheduled (e.g., December 4th 5:00 PM to December 7th 8:00 AM).
  
• Complete required orientation and submit necessary documentation
  
• Adhere to all facility policies and credentialing requirements.
