Key Responsibilities

• Conduct thorough evaluations of patients, utilizing a variety of assessment tools to gather their health history, perform physical assessments, and determine treatment recommendations based on findings.
• Communicate evaluation results and treatment plans to patients and their families, ensuring their engagement in goal-setting and decision-making processes.
• Implement treatment protocols that are tailored to individual patients reflecting their unique presentations and needs, while adapting to changes in their conditions as required.
• Collaborate with other health professionals to optimize patient care and document any modifications to the treatment plan as necessary.
• Manage patient discharge planning effectively, ensuring that all follow-up care instructions are clear and understood to promote continued recovery.
• Maintain accurate and timely documentation of patient encounters, including initial evaluations, daily treatment notes, progress summaries, and discharge evaluations, in alignment with accreditation standards.
• Demonstrate proficient time management skills and ensure adherence to productivity expectations and departmental objectives.
• Engage in continuing education to advance your physical therapy skills and respond to evolving industry standards.
• Actively participate in marketing initiatives within the community to enhance the visibility of our services and foster relationships with potential patients.

Patient Population / Client Focus

Our patients represent a wide range of demographics and include individuals recovering from surgeries, sports injuries, and chronic pain conditions, among other concerns. As a Physical Therapist at Upstream Life, your primary focus will be on guiding these patients through their rehabilitation journey, harnessing your skills to facilitate recovery and improve functional outcomes. You will also have access to diverse therapeutic modalities tailored to meet the specific needs of different age groups and abilities.

Qualifications

• Bachelor's Degree in Physical Therapy; a Doctorate in Physical Therapy (DPT) is preferred.
• Valid Physical Therapy license in the State of Florida.
• Current CPR certification through the American Heart Association's BLS for Healthcare Providers.
• A minimum of 2 years of clinical experience in physical therapy is required.

Preferred Experience

• Experience in outpatient rehabilitation settings is highly desirable.
• Familiarity with modern treatment modalities and electronic health record (EHR) systems.

Compensation and Benefits
