Health Access Network seeks a dedicated full-time Licensed Clinical Social Worker (LCSW) to provide critical services in our Hospice Center, located in Norfolk, Virginia. This remote position allows for flexibility while ensuring that our vulnerable populations receive the support they need during challenging times. Work Environment\n

This role is primarily remote, providing the flexibility to serve clients effectively while balancing personal work-life commitments. Although the majority of your work will be conducted from home, there may be occasions where in-person meetings or community outreach events take place, requiring you to be physically present. Key Responsibilities\n

• Conduct comprehensive assessments to identify clients’ social, emotional, and environmental challenges. This may involve evaluating the client’s living situation, mental health status, and any immediate risks they may face.

• Develop individualized service plans with detailed goals and interventions, guiding each client through the steps required to access essential support and resources.

• Provide empathetic counseling and support to individuals and families as they navigate life-altering challenges, helping to build resilience and coping strategies.

• Collaborate actively with clients to ensure they are connected with relevant community resources, such as housing assistance, mental health services, and addiction recovery programs.

• Offer crisis intervention, including risk assessments and safety planning for clients experiencing acute distress or emergencies.

• Engage with community agencies and organizations to advocate for clients effectively, ensuring they receive necessary services and support.

• Participate diligently in community outreach activities designed to raise awareness about our services and promote engagement within the community.

• Contribute to case conferences and team meetings, sharing insights and strategies while also participating in ongoing training to enhance professional development.

Patient Population / Client Focus\n

You will be working primarily within our hospice center, engaging with individuals and families facing end-of-life challenges. This sensitive population may include terminally ill patients and their loved ones who require compassionate support and guidance as they navigate grief, decision-making, and emotional distress. You will play a crucial role in offering not only practical assistance but also emotional support, helping clients find peace and comfort in this difficult time.

Qualifications\n

• A Master's degree in Social Work (MSW) from an accredited program is required.

• Active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.

Preferred Experience\n

Compensation and Benefits\n
