Key Responsibilities\n

• Conduct thorough psychosocial assessments to understand clients' mental health needs, strengths, and resources.

• Provide individualized therapy sessions for clients, families, and groups utilizing evidence-based approaches tailored to hospice care.

• Implement therapeutic modalities such as cognitive-behavioral therapy, solution-focused therapy, and trauma-informed care while being sensitive to the unique contexts of terminal illnesses.

• Develop, monitor, and adjust personalized treatment plans with clear goals and interventions based on ongoing assessments.

• Collaborate closely with the multidisciplinary care team to ensure seamless communication and coordination of care.

• Engage in crisis intervention when necessary, conducting risk assessments and safety planning to ensure clients' well-being.

• Document all interactions and treatment outcomes in compliance with ethical standards and legal regulations.

• Facilitate psychoeducation classes, group therapy sessions, and support groups, providing education about coping mechanisms and available resources to clients and their families.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• Active state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
