Work Environment

This position is based onsite as you will serve patients in their own homes throughout Clearwater, Florida. Our home care physical therapy model allows you to build meaningful relationships with patients and their families, providing care that is both specialized and flexible. Key Responsibilities

• Develop and implement individualized treatment plans aimed at preventing disability, alleviating pain, improving muscle function, and maintaining optimal performance as guided by physician prescriptions.
• Educate and motivate clients in performing prescribed exercises and functional activities, emphasizing a hands-on approach to recovery.
• Conduct detailed evaluations of clients' physical capabilities, including diagnostic assessments of muscle, joint, nerve, and functional abilities.
• Supervise the implementation of treatment plans by Physical Therapy Assistants, ensuring adherence to best practices and adjustments as necessary.
• Investigate home environments to identify potential hazards and provide recommendations that foster independence in daily living activities.
• Administer therapeutic techniques, including manual exercises, traction, and massage, while utilizing physical agents to enhance recovery.
• Document clinical evaluations, progress reports, and discharge summaries accurately to contribute to the client's health records and treatment plans.
• Actively participate in interdisciplinary team meetings to devise a comprehensive treatment strategy for each client.
• Establish a reliable line of communication with clients, families, and medical personnel, ensuring everyone involved in the healthcare journey is informed and engaged.
• Maintain timely documentation and follow-ups as mandated by branch policies regarding treatment orders and progress.”

Patient Population / Client Focus

In this role, you will primarily work with individuals who require rehabilitative care in their homes, including elderly patients recovering from surgeries, individuals managing chronic conditions, and those who have recently experienced significant health challenges. Your approach will be grounded in empathy and tailored to each patient's unique needs, fostering an environment of trust and support.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited program.
• Active Physical Therapist license in good standing for the state of Florida.
• Current CPR certification to ensure the safety and emergency preparedness of patients.
• Strong communication skills and a talent for building rapport with patients of diverse backgrounds.
• A professional attitude and an ongoing commitment to excellence in patient care.

Preferred Experience

• At least one year of experience in a physical therapy setting, preferably with exposure to home care services.

Compensation and Benefits

The salary range for this position is competitive, offering between USD 100,000 and 120,000 per year.
