Break Through Rehab, LLC is seeking an experienced and dedicated Physical Therapist to join our team in Clearwater, Florida. This full-time, onsite position offers a unique opportunity to deliver personalized physical therapy services within patients' homes, making a tangible impact on their recovery and quality of life. This position is fully onsite, with the Physical Therapist providing care in the homes of patients across Clearwater, Florida. As a Physical Therapist at Break Through Rehab, you will engage in the following key responsibilities:

Review and analyze patients' medical histories to ascertain their health needs and formulate appropriate treatment strategies.

Consult with patients to identify their physical conditions and discuss symptoms, ensuring they understand the rehabilitation process.

Administer modalities such as massage and electrical stimulation to enhance healing and pain management.

Document patient progress meticulously, tracking rehabilitation goals and modifying treatment as needed.

Communicate with multidisciplinary healthcare providers to coordinate comprehensive care for patients.

Our Physical Therapists serve a diverse patient population, primarily focusing on individuals recovering from orthopedic surgeries, accidents, or chronic conditions. You will collaborate with elderly patients, individuals with developmental disabilities, or those experiencing post-surgical rehabilitation, requiring a compassionate approach tailored to varied backgrounds and conditions.

A Bachelor's Degree from an accredited Physical Therapy program, although a Master's Degree is preferred.

A valid state licensure to practice Physical Therapy.

A minimum of 3 years professional experience in physical therapy, specifically in orthopedic settings.

Proficient in record-keeping with solid computer and word processing skills.

In addition to the required qualifications, candidates with experience in home health care or those who have worked with geriatric populations will be given preference. Familiarity with telehealth practices or a history of integrating technology into patient care will also be viewed favorably.
