Job Opportunity

Position Overview

The role involves coordinating and directing home nursing care based on physician orders, adhering to agency policies and procedures while utilizing the nursing process effectively. The nurse will be responsible for supervising and evaluating Home Health Aides as well as Licensed Practical Nurses/Licensed Vocational Nurses.

Work Schedule

Shift: Day 5x8-Hour (08:00 AM - 05:00 PM)

Responsibilities

Provide competent home nursing care following physician's orders.
Oversee and evaluate the performance of Home Health Aides and LPNs/LVNs.
Conduct home visits to deliver care to patients.
Attend periodic office meetings as required.
Complete documentation within 24 hours of patient appointments.
Ensure adherence to agency policies and local regulations.

Travel Requirements

Position requires home visits with the expectation of travel up to 75 miles from the home clinic. Reliable transportation, a current driver's license, and agency-required liability insurance are mandatory.

Patient Care

Average number of patients per day: 5-10.

Wound Care
Follow Up Visits

Qualifications

Minimum of 1 year of Adult Home Health experience or Hospice experience, based on specialty.
Must possess current licensure to practice as a registered nurse.
CPR and BLS certifications are required.
Proficiency in HomeCare HomeBase EMR and OASIS is mandatory, with recent experience within the last 6 months.

Additional Information

All documentation must be completed accurately and within the specified timeframe to ensure quality patient care and compliance with billing standards. The expectation is that the nurse will accept all scheduled patients, regardless of geographical distance between appointments. Declining patients may affect contract status.
