Key Responsibilities\n

• Conduct thorough assessments to evaluate patients' psychosocial needs and develop tailored treatment plans that may include short-term counseling and crisis intervention strategies.

• Guide patients through the process of obtaining necessary entitlements and benefits, assisting them in completing forms for supplemental insurance, ensuring they have access to services that reduce dependency on emergency programs.

• Facilitate access to financial assistance for prescriptions by connecting patients with pharmaceutical company indigent programs and community resources.
\n
• Advocate on behalf of patients, partnering with community agencies to enhance access to needed services and resources.
\n
• Maintain accurate and up-to-date patient records, ensuring compliance with documentation standards and preparing reports as required by organizational policies.

• Coordinate transportation assistance for patients when necessary, ensuring they can attend appointments and access community services.

• Participate in clinical meetings and team discussions to contribute to case management strategies and enhance treatment efficacy.

Qualifications\n

• A Master's degree in Social Work, Psychology, Marriage and Family Therapy, or Counseling is required.

• Possession of one of the following active licenses: Licensed Professional Clinical Counselor (LPCC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT).

• A minimum of two (2) years of professional experience in mental health counseling is mandatory.

Preferred Experience\n

• Experience working in an outpatient setting is preferred.

• Familiarity with case management workflows and community resource navigation will be beneficial.

• Proficiency in electronic health record systems for documentation and patient tracking is a plus.

Compensation and Benefits\n
