This full-time, remote position is focused on providing critical support to seniors in a hospice setting. Work Environment\n

This role offers a flexible working environment, allowing you to carry out assessments and support clients in the comfort of their homes. While primarily remote, you will be required to conduct in-person visits at our hospice center and clients’ residences, making effective time management and scheduling flexibility essential. Work hours are typically Monday through Friday, from 8:00 AM to 4:30 PM, with occasional evenings or weekends based on client needs.

Key Responsibilities\n

• Conduct comprehensive assessments of clients’ psychosocial needs, considering emotional, social, and mental health factors.

• Develop personalized care plans in collaboration with clients and their families, focusing on desired outcomes and unique preferences.

• Provide counseling and emotional support to seniors and their families to assist them in managing transitions, such as end-of-life care.

• Advocate for clients' rights and ensure their access to appropriate resources, including healthcare services and social support systems.

• Collaborate with a multidisciplinary team of medical professionals to coordinate care and optimize service delivery.

• Maintain ongoing client contact through regular visits, ensuring their situation is continually assessed and care plans are updated accordingly.

• Facilitate family meetings to discuss care challenges and promote effective communication among family members.

• Educate clients and their families about community resources, support networks, and programs that enhance social engagement.

• Document client interactions, care plans, and progress notes accurately and in a timely manner using electronic health record (EHR) systems.

• Stay current on best practices and ethical standards in social work to ensure compliance and quality of care.

Your primary focus will be seniors receiving hospice care and their families. You will engage with individuals facing complex emotional and social dynamics related to end-of-life issues, requiring a firm understanding of geriatric care, empathy, and the ability to navigate sensitive conversations. Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Active and valid state licensure as a Licensed Master Social Worker (LMSW).
