Work Environment\n

This position is entirely remote, enabling flexibility while working from home. As part of a hospice center, your work setting will typically involve interdisciplinary collaboration via digital platforms. Key Responsibilities\n

As a Licensed Clinical Social Worker, your day-to-day responsibilities will include:

• Conducting comprehensive evaluations and assessments of children to identify support needs.
\n
• Participating in the development and implementation of Individualized Family Service Plans (IFSPs) tailored to the specific needs of each child.
\n
• Monitoring progress and outcomes, serving as an active member of a multidisciplinary team focused on the child's development.
\n
• Engaging with families to provide educational resources and emotional support, facilitating effective communication and understanding of their child's development.
\n
• Utilizing evidence-based practices and Natural Learning Environment Practices to build rapport and create a nurturing coaching model that is individualized to each child and family.
\n
• Developing joint plans with families to recommend strategies that enhance their child's development while complying with all applicable Federal and State regulations.
\n
• Ensuring the health and safety of staff and the children served by adhering to established health standards.

Patient Population / Client Focus\n

You will primarily work with young children, including infants and toddlers, who may have disabilities or developmental delays. Your role will directly impact their growth in key skill areas including physical, cognitive, social-emotional, and communication skills. Through comprehensive assessment and intervention, you will contribute to improving their quality of life and overall development.

Qualifications\n

The ideal candidate will possess the following qualifications:

• A Master’s Degree or Doctorate in Social Work from an accredited institution.

• Current licensure as a Licensed Clinical Social Worker (LCSW) by the Department of Public Health.

• A valid driver’s license.

Compensation and Benefits\n
