Work Environment\n

This position is remote, allowing for flexibility in work hours while collaborating with a multidisciplinary team. You will work closely with healthcare professionals, families, and specialists from various domains. Key Responsibilities\n

• Conduct thorough evaluations and assessments to understand the needs of children within the hospice program.

• Actively participate in developing and implementing Individualized Family Service Plans (IFSPs) that align with the unique needs of each child and family.

• Monitor and report on progress, adapting strategies as necessary to achieve desired outcomes.
\n
• Facilitate meaningful support to families in navigating challenges, offering guidance and resources to promote healthy development.
\n
• Administer evidence-based evaluations and communicate results effectively with families and team members.
\n
• Foster relationships with children and their families to encourage a trusting and healthy learning environment.

• Utilize a coaching model and natural environment teaching strategies to enhance children’s development.
\n
• Ensure adherence to relevant Federal and State regulations and protocols to maintain quality and compliance.

Patient Population / Client Focus\n

Your focus will be on supporting families with children who have disabilities or are in hospice care, helping them to learn and grow during critical early years. Working closely with pediatric patients and their families, you will facilitate a compassionate approach that respects the unique experiences of each child and promotes their emotional, social, and developmental well-being.

Qualifications\n

• A Master's degree or Doctorate in Social Work, with a valid LCSW license from the Department of Public Health.

• Confident in adapting treatment modalities to meet the diverse learning styles and individual treatment plans of children.

• Demonstrable experience working in early intervention settings for at least one year.

• Proficient communication skills for effectively engaging with children and collaborating with team members.

• A valid driver’s license to facilitate engagement with families as needed.
