Key Responsibilities\n

• Conduct comprehensive psychosocial assessments through detailed interviews, keen observations, and thorough reviews of medical records to identify emotional, social, and environmental factors affecting patient recovery.

• Provide direct counseling and emotional support to patients and their families, addressing common adjustment issues associated with rehabilitation and promoting effective coping strategies.

• Collaborate closely with rehabilitation professionals, educators, and support staff to develop and implement individualized treatment plans based on each patient's unique psychosocial needs.

• Assist patients in navigating and accessing community resources, such as support groups, vocational rehabilitation services, and housing assistance, to facilitate a smooth transition back into their everyday lives.

• Support discharge planning efforts, ensuring comprehensive communication with families about post-rehabilitation services, community resources, and the next steps in their care journey.

• Utilize telehealth platforms when necessary, ensuring that services remain accessible to all patients regardless of their situation.

Qualifications\n

Compensation and Benefits\n
