Key Responsibilities\n

• Conduct thorough psychosocial assessments through structured interviews, observational methods, and a detailed review of relevant medical and educational records to evaluate emotional, social, and environmental factors affecting students.

• Provide individual and group counseling sessions to students and their families to facilitate adjustment to challenges related to their health and recovery.

• Work in close collaboration with the rehabilitation team to formulate individualized treatment plans that align with the students’ psychosocial needs, ensuring a well-coordinated approach.

• Assist students in identifying and accessing community resources such as support groups, vocational rehab, and housing assistance, amplifying their chances of recovery and adjustment.

• Educate families about available community resources and post-rehabilitation services to foster a network of ongoing support.

• Participate actively in discharge planning, preparing students and their families for successful transitions back to home or other care settings.

Qualifications\n

• Master's degree in Social Work from an accredited institution.
