Job Overview Registered Nurse (RN) – Home Health. Location: Proviso, Illinois. Duration: Ongoing. 40 hours per week, 8‑hour shift. Staff position. Responsibilities Lead and collaborate as a case manager to deliver and manage patient care plans. Provide home health nursing care to patients in Elmhurst, Bensenville, and surrounding areas. Meet charting deadlines and maintain accurate documentation. Travel to patient sites (~20% of time) and manage time and schedule independently. Use tablets, mobile phones, and EMR software for patient care. Qualifications Registered Nurse license in the state of employment. Graduate of an approved school of practical nursing. Minimum one year of RN experience with clinical competence to deliver quality patient care. Current driver’s license with ability to drive to patient locations.
