Hospice Liaison
Location: Minneapolis, MN

Summary

Full Spectrum Search Group is a full-service healthcare recruiting agency specializing in post-acute leadership and healthcare administration. We are currently conducting a search for a Hospice Liaison on behalf of a national hospice provider in the Minneapolis, MN market.

Key Responsibilities

• Build and maintain referral relationships with physicians, hospitals, skilled nursing facilities, and community partners
• Educate referral sources and the community on hospice services and eligibility
• Manage and grow a designated territory through strategic outreach and partnership development
• Collaborate with intake, admissions, and clinical teams to ensure a seamless patient onboarding process
• Track outreach activity and performance using CRM systems and territory planning tools
• Contribute to a high-performing, ethical sales culture centered on patient outcomes

Qualifications

• Experience in healthcare sales
• Hospice, home health, or post-acute care experience strongly preferred
• Demonstrated success in sales performance and relationship building
• Established network within the Minnesota healthcare market preferred
• Strong communication, presentation, and interpersonal skills
• Experience managing a territory and utilizing CRM tools
