Knapp Rehabilitation Center Admission Liaison

The Knapp Rehabilitation Center Admission Liaison exists to grow and facilitate the admissions of individuals to the Knapp Rehab Center both within Hennepin Healthcare and with our external partners. This position is the "face" of Knapp Rehab for internal and external referring organizations, predominantly, acute care hospitals throughout the Twin Cities area. The Liaison must build productive professional relationships and is often the first impression patients, families and referral sources experience with Knapp Rehab. This position serves as a link between referral sources (both internal and external) with the goal of efficiently and effectively transitioning individuals into their inpatient rehabilitation episode of care.

Responsibilities include:

• Building productive professional relationships, representing and selling Knapp Rehab services to various audiences within the referral sources, primarily physicians, hospital discharge planners/case managers, patients and families
• Supporting the admission process, facilitating a smooth conversion from referral to admission by assessing the admission appropriateness and providing clinical information to Knapp Rehab while communicating effectively with all parties
• Executing market development tactics to identify and grow new business opportunities and volume
• Demonstrating deep knowledge of market dynamics as it pertains to rehabilitation services and post-acute providers in the Minneapolis Market
• Meeting or exceeding the assigned admission goals - annual, quarterly and monthly - for rehabilitation services by establishing and maintaining productive referral relationships with key referrers and clinical decision makers
• Serving as the primary contact regarding services at Knapp Rehab Center to the targeted referral sources - for all the various customer audiences including, but not limited to physicians, case managers, discharge planners, social workers, therapists, and key nursing roles
• Creating and maintaining Knapp Rehab presence for all targeted referral sources according to the annual plan which may include certain planned communication and public relations-related activities to specific audiences and accredited educational programs to various audiences
• Scheduling ongoing in-services and professional presentations for referral sources, community groups and other organizations regarding Knapp Rehab services programs and patient outcomes as needed
• Maintaining an up-to-date comprehensive understanding of the acute care and rehabilitation market in the assigned region including a detailed understanding of the rehabilitation market competitors
• Determining medical appropriateness of patients and supporting the admission process by gathering and transferring medical information to the inpatient rehabilitation admitting team
• Meeting with families/patients at the referral source to educate them on the benefits of coming to the Knapp Rehab Center and to ensure the referral becomes an admission
• Recognizing barriers to admission, documenting these barriers, responding appropriately and working with the Knapp Rehab leadership to follow up on barriers to access
• Preparing regularly scheduled or special reports; collecting and analyzing data that is the basis for assigned reports
• Supporting and participating in exhibits at scheduled clinical conferences
• Managing the administrative aspects of referral sources by ensuring the databases accurately reflect the referral/admission activities and maintaining a current listing of all referral relations activities in the assigned contact management system
• Performing all other duties that may be assigned in the best interest of the Knapp Rehab Center
