Key Responsibilities\n

• Conduct and document comprehensive bio-psychosocial assessments regarding patients' mental health, substance abuse, and medical needs, allowing for a holistic understanding of their situation.

• Engage with individuals, couples, and families, providing a variety of behavioral health services such as counseling, intake assessments, diagnosis, crisis intervention, and treatment planning.

• Facilitate referrals to local resources including community agencies and medication assistance programs to develop an integrated support network for patients.

• Maintain accurate and timely electronic documentation of mental health evaluations, progress notes, and any necessary treatments, ensuring compliance with regulations and improving patient outcomes.

• Collaborate with quality improvement teams to establish effective data collection processes that enhance service delivery and patient care initiatives.

Patient Population / Client Focus\n

The primary patient population includes individuals in hospice care, as well as their families. Many patients may be experiencing a variety of psychological challenges, such as anxiety, depression, or existential distress related to terminal illness. Qualifications\n

• Must possess a valid state Professional License as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
