Field Care Coordinator - Medicaid & Community Resources

Humana Inc

Humana Inc. is seeking a Care Coordinator in Illinois to support Medicaid Members within the Illinois HealthChoice Medicaid Managed Care program. You will assess needs, develop care plans, and connect families with resources to improve health and well-being. The role involves home visits or facility visits with 75-90% local travel, coordinating services, and ensuring progress toward care goals.
