This role is completely remote, providing flexibility and the convenience of a home-based work setting. However, the Social Worker will be required to travel to various CHDC locations and participate in in-person meetings, workshops, and outreach functions as necessary. Key Responsibilities\n

As a Licensed Clinical Social Worker at CHDC, your primary duties will include:

• Delivering high-quality clinical behavioral health services in alignment with the health center's mission to enhance overall patient health outcomes and satisfaction.

• Guiding mental health clients through the complexities of physical and behavioral healthcare systems, ensuring they receive integrated and comprehensive care.

• Educating patients on available community resources and services, empowering them to participate in disease prevention and wellness initiatives.
\n
• Actively participating in multidisciplinary treatment team meetings to contribute to cohesive care planning.
\n
• Reporting all crisis situations and unusual incidents promptly to the Medical Director, and ensuring immediate escalation of psychiatric or medical emergencies to the Chief Medical Officer (CMO).
\n
• Systematically documenting clinical interactions and maintaining accurate patient records as per organizational guidelines.
\n
• Engaging in Quality Improvement programs as assigned, participating in ongoing training sessions to enhance service delivery.
\n
• Upholding compliance with all organizational policies and procedures, and serving as part of a cohesive integrated Primary Care Behavioral Health team.
\n
• Utilizing evidence-based screening tools to assess the healthcare needs of individual patients and ensuring proper reporting of findings.
\n
• Collaborating with specialized teams, particularly in cases involving patients with Opioid Use Disorder, to provide tailored therapeutic interventions.

Patient Population / Client Focus\n

Your focus will be on serving a diverse range of patients who have varying mental health needs. This includes individuals facing challenges related to mental health disorders, addiction, and those requiring support in navigating community resources. Your role will be vital in providing culturally competent care and building trusting relationships within the community.

Qualifications\n

• A Master's Degree in Social Work is required, and you must hold a valid Licensed Clinical Social Worker (LCSW) certification.

• A minimum of 2-3 years of relevant experience in social work, particularly within a healthcare environment.

• Demonstrated ability to work effectively in collaboration with other healthcare professionals and social service providers.

• Excellent communication and interpersonal skills to engage with clients empathetically and effectively.

• This position requires the readiness to travel to multiple CHDC sites for meetings and engagements.

Preferred Experience\n

• Previous experience working in a primary, specialty, or integrated care setting is highly preferred.

• Familiarity with evidence-based practices and tools used for assessing and treating mental health conditions.

• Experience engaging with patients dealing with substance use disorders, particularly in coordination with a Center of Excellence Team.
