Key Responsibilities\n

• Conduct comprehensive assessments to understand the unique needs of each patient, tailoring therapy approaches to their specific circumstances.

• Maintain ongoing communication with supervisors, family members, and other stakeholders to discuss patient progress and treatment plans.

• Create and manage a tentative schedule for therapy sessions, home visits, and observations to monitor patient development effectively.

• Organize and perform home visits as required for thorough evaluations, including chart reviews, staff interviews, and data collection.

• Implement treatment plans with fidelity, ensuring clinical interventions are effective and aligned with best practices.

• Participate actively in interdisciplinary team meetings, contributing insights regarding patient care and program effectiveness.

• Document and complete monthly and quarterly progress notes to track patient outcomes and inform ongoing treatment strategies.

• Provide training and support to new staff members and team peers, ensuring they are well-equipped to implement program plans and maintain accurate documentation.

• Offer follow-up interviews and home visits to ensure continued support for patients, addressing any emerging needs promptly.

• Maintain organized records to ensure information is current and readily accessible to authorized personnel.

• Act as a liaison between the public and the mental health community, working to enhance program capabilities and public awareness.

Qualifications\n
