The Legacy at Boca Raton Rehabilitation and Nursing Center is seeking a dedicated and compassionate Physical Therapist (PT) to join our rehabilitation team. The ideal candidate will be responsible for evaluating residents, developing individualized treatment plans, and providing high-quality therapy services to help residents achieve their highest level of independence and functional mobility.

Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans
• Provide therapeutic exercises, mobility training, balance training, and pain management interventions
• Monitor resident progress and adjust treatment plans as needed
• Collaborate with physicians, nursing staff, and interdisciplinary team members to ensure optimal resident outcomes
• Maintain accurate and timely clinical documentation in accordance with regulatory and facility requirements
• Educate residents and families regarding therapy programs and treatment goals
• Ensure compliance with all federal, state, and facility policies and regulations
• Participate in care planning meetings and discharge planning processes
• Maintain a safe and supportive therapy environment

Qualifications

• Active Florida Physical Therapist (PT) license in good standing
• Previous experience in a skilled nursing facility, rehabilitation center, or healthcare setting preferred
• Strong clinical assessment and treatment planning skills
• Excellent communication and interpersonal abilities
• Ability to work collaboratively within an interdisciplinary team
• Compassionate and resident-focused approach to care
