Work Environment

The work schedule typically runs from Monday to Friday, offering flexibility with hours from either 9 AM to 5 PM or 8 AM to 4 PM. Our home care facility encompasses a state-of-the-art setup that allows physical therapists to implement innovative techniques in real-world contexts, reaching out to patients in the comfort of their homes.

Key Responsibilities

• Conduct thorough evaluations of patients' physical conditions, identifying functional limitations and creating individualized treatment plans that address their unique needs.
• Utilize an array of techniques, including therapeutic exercises, massage, and stretching to enhance mobility, alleviate pain, improve balance, and restore functionality.
• Lead clinical teaching initiatives, providing mentorship and instruction to interns, PT students, and staff, while ensuring the educational needs of the team are met.
• Engage in the initiation of clinical research under the guidance of management to advance clinical practices and methodologies.
• Collaborate with multidisciplinary teams to ensure integrated care approaches and optimal patient outcomes.
• Maintain detailed patient documentation and progress notes consistent with organizational and regulatory standards.

Patient Population / Client Focus

Your primary focus will be on a diverse outpatient population, primarily consisting of individuals recovering from orthopedic surgeries, injuries, or conditions affecting their mobility and functionality. This role allows you to build meaningful relationships with patients as you guide them through their rehabilitation process, helping them regain strength, balance, and independence.

Qualifications

• Required: Bachelor's Degree in Physical Therapy from an accredited educational institution.
• Active New York State Physical Therapy license.
• Minimum of 2 years' experience working as a physical therapist, specifically in orthopedic care.
• Strong clinical assessment and treatment skills, coupled with the ability to create tailored and actionable rehabilitation plans.
• Exceptional communication and interpersonal abilities, enabling effective interaction with patients and healthcare teams.

Preferred Experience

• Board certification in Orthopedic Physical Therapy is highly desirable.
• Experience with home health care settings and familiarity with patient-centered rehabilitation in non-clinical environments.
• Knowledge of electronic health record (EHR) systems for documenting patient progress and treatment outcomes.
