Healthcare Care Coordinator

Our client is seeking a highly motivated and empathetic individual to join our team as a Care Coordinator. In this critical role, you will be the first point of contact for patients, physicians, and referral sources, providing compassionate support and efficient, first-call resolution.  Location: Remote Hours: Must be flexible between 7:00 a.m. to 8:00 p.m. CST Rate: $20.00 per hour Please apply online for consideration

Responsibilities

• Create and complete accurate referrals and applications using our centralized Customer Relationship Management (CRM) platform, staying updated on any policy or procedural changes.
• Resolve patients' questions and concerns regarding their patient journey status with clarity and empathy.
• Enter detailed, accurate, and clear information into our company proprietary software while conversing with patients via telephone.
• Place outbound phone calls for patient follow-ups or confirmations to ensure timely progression of their accounts.
• Maintain high-quality service while providing an empathetic and supportive experience to the patient by confidently controlling the conversation and clearly educating the caller.
• Interact with patient referral sources, which may include healthcare providers, to efficiently process new applicants.
• Take ownership of patient accounts from initial contact through final approval or denial, ensuring a seamless experience.
