Key Responsibilities\n

• Conduct ongoing assessments to accurately determine the needs and goals of each patient.

• Facilitate clear communication between supervisors, healthcare staff, families, and guardians to support the individual’s treatment progress.

• Organize a tentative schedule for appointments, home visits, and follow-up observations to ensure consistent care and support.
\n
• Plan and coordinate in-home visits that include chart reviews and staff interviews as part of comprehensive data analysis.
\n
• Ensure that all elements of the treatment program are implemented effectively and adjusted as necessary based on patient feedback and progress.

• Actively participate in Interdisciplinary Team Meetings and collaborate with other professionals to enhance patient outcomes.
\n
• Document patient progress through comprehensive monthly and quarterly reports.
\n
• Coordinate training sessions for staff to ensure competency in implementing program plans and appropriate documentation practices.

• Provide ongoing follow-up support through interviews, home visits, and observations tailored to meet ongoing patient needs.

• Maintain organized and accurate records to ensure accessibility for all relevant parties involved in the patient’s care.

• Serve as a liaison between the mental health community and the public to advance community wellness initiatives.

• Contribute to the development of training materials aimed at improving programs and services offered to individuals.

Patient Population / Client Focus\n

As a Licensed Professional Clinical Counselor at Madina Care Consulting, you will work with a diverse clientele, including both adults and youth, addressing a wide array of mental health challenges such as anxiety, depression, trauma, and substance misuse. Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field.

• Active and unrestricted licensure as a Professional Clinical Counselor (LPCC) in the state of California.

Compensation and Benefits\n

The salary range for this position is competitive, ranging from USD 90,000 to 100,000 per year, commensurate with experience and caseload volume.
