Key Responsibilities\n

• Conduct comprehensive assessments to evaluate and support the individual needs of patients, ensuring a tailored therapeutic approach.

• Maintain clear and effective communication with supervisors, healthcare team members, guardians, and family members concerning client progress and treatment plans.

• Schedule and oversee appointments, home visits, and therapeutic observations, providing ongoing oversight of client development.

• For clients requiring in-home support, coordinate home visits to facilitate thorough observations and collect pertinent data through chart reviews and staff interviews.

• Implement program plans effectively and monitor adherence, ensuring each client receives appropriate interventions.
\n
• Participate actively in Interdisciplinary Team Meetings and other relevant collaborative forums that support coordinated client care.
\n
• Document client progress through concise monthly and quarterly notes, reflecting therapeutic progress and treatment adjustments.
\n
• Lead and organize training sessions to ensure all staff members are adept at implementing the program plans and maintaining accurate documentation.
\n
• Conduct follow-up interviews and additional home visits to ensure clients receive continuous support based on their evolving needs.

• Maintain comprehensive client records to enable informed care decisions and facilitate access for authorized parties.

• Act as a liaison with the public and broader mental health community to advance development goals and enhance resource connections for our clients.

• Assist in the development and refinement of training materials aimed at improving program efficacy for client care.

Qualifications\n
