Key Responsibilities\n

• Provide expert counseling and support to patients and families, addressing their emotional and psychological needs related to illness.

• Maintain a comprehensive knowledge of government and private payer networks to ensure effective care transitions and optimal patient outcomes.

• Coordinate with community resources and healthcare professionals to develop individualized care plans that enhance the overall well-being of patients.

• Act as an advocate for patients, supporting their rights and advocating for access to protective services, as necessary.
\n
• Ensure compliance with legal requirements regarding reporting vulnerability or suspected abuse.
\n
• Participate in transition conferences with patients and families to facilitate care continuity.

• Utilize telehealth tools and technology to monitor, document, and communicate patient progress and social work plans efficiently.
\n
• Identify patients at risk for readmission and proactively implement interventions to prevent this occurrence.
\n
• Engage in regular communication with an interdisciplinary team to ensure an integrated approach to care delivery.
\n
• Document and maintain accurate records of patient interactions and treatment plans through electronic medical records (EMR).
\n
• Provide financial and social resource assistance information to patients and families, addressing their specific needs.
\n
• Fulfill additional duties as assigned by management to support team objectives and patient care delivery.

Qualifications\n

Compensation and Benefits\n
