Key Responsibilities\n

• Counsel and conduct crisis interventions for patients facing emotional and behavioral challenges, carefully assessing their needs and crafting individualized treatment plans.

• Maintain ongoing communication with medical professionals, including physicians and psychiatrists, to report patient progress, address concerns, and develop integrated care solutions.

• Ensure accurate documentation by maintaining comprehensive clinical records and progress notes in compliance with organizational policies and electronic medical records systems.

• Collaborate closely with a multidisciplinary health care team to craft and revise treatment and discharge plans based on detailed psychosocial assessments.

• Actively represent New Dawn Wellness in community resource groups, forging partnerships and advocating for the social needs of patients in meetings and committees.

Qualifications\n
